Qualifying documentation requirements:

  • A letter or a document that states the diagnosed disability from a professional evaluator qualified to determine the stated diagnosis. For example, a physician, psychologist or psychiatrist, audiologist, ophthalmologist, licensed psychological counselors, school psychologists, etc.
  • Any documentation must be dated within the past 3 years

In order for a high school IEP or 504 plan to be sufficient, it will need to include a confirmation of the diagnoses, the name of the evaluator(s), and the accommodations the student received. You do not need to submit the entire IEP or 504 plan, only the pages that contain the previously listed information. Please be aware that approved accommodations from other academic settings do not automatically transfer to BRCC. Additional documentation may be requested after initial review by Disability Services.
